LDA West Regional Project Coordinator (Social Services Professional 1 - MR07)

The LDA West Regional Project Coordinator will coordinate the implementation of the Intrastate Network to Deliver Equity and Eliminate Disparities (INDEED) grant in the School of Nursing. The position will be remote, preferably located in West Tennessee, and will require routine daily travel, rarely overnight.
Duties and Responsibilities:
Outreach and Recruitment
Procure relationships with faith and religious community stakeholders in West Tennessee; support faith and religious leaders as they recruit congregation members to attend training sessions for ongoing health education and care navigation; plan and host events such as community health fairs and education workshops; assist with planning and hosting a statewide educational and networking conference; identify regional resources to include in community resource network; and collaborate with the Tennessee Department of Health and UTC to develop and disseminate grant-related marketing materials.
Assessment
Collaborate with regional stakeholders to complete a needs assessment of faith-based communities in the West Tennessee region; and contribute to the data collection, analysis, interventions, and evaluation process related to the needs assessments.
Implementation
Assist faith and religious congregations by implementing an action plan based on their identified needs; and collaborate with partner organizations to develop, assimilate, and present educational content related to community health and resources related to memory loss, caregiving, chronic disease prevention and management, infection prevention, wellness, safety and the social determinants of health.
Care Navigation
Collect resources and contribute to a statewide resource directory for care navigation; and collaborate with identified partner resource organizations to increase accessibility and knowledge of services they offer.
Evaluation
Assist with collecting, analyzing and presenting program data and reports; attend weekly grant meetings and provide a summary of progress to the grant team; and communicate routinely with principal investigator to ensure program outcomes and deliverables are met, and to troubleshoot barriers when indicated.
The ideal candidate will possess the following:
- Proficiency in Excel and Microsoft Office/Office 365.
- Strong interpersonal and communication skills.
- Knowledge of community health, health-disparities and faith-based communities.
This position is a Limited Duration Appointment (LDA).
A Limited Duration Appointment (LDA) is a form of regular employment for a specific project or for a limited period. An LDA may be established for up to one year with an option to renew the appointment annually for a maximum of three years. Please note that this position may be converted to a permanent position prior to or at the conclusion of three years, funding dependent.

Minimum qualifications: High school diploma or GED and 2 years of work experience in community health or a faith-based organization.
Preferred qualifications: Associate's degree and 1 year of experience in grant management; experience with federal or state grants and/or higher education; residence in West Tennessee; and bilingual in English and Spanish.
